The header page is where the core details (top section) of the assessment are held.  This includes;

 

1.Risk Assessment Unique Reference ID

2.The Site or Location applicable to the risk assessment

3.The risk assessment details (the task being performed)

 

The header page always remains visible regardless of what tab the user moves to.

 

Below the header section the first tab allows for additional header details to be added, such as;

 

1.Date and time risk assessment undertaken

2.Number of employees covered by risk assessment

3.Name of risk assessor and any persons assisting

4.Date when risk assessment should be reviewed by

5.The option to state risk assessment review not required (will remove risk assessment from several monitoring procedures and various reports)
