The opening line of clause 7.5.2 defines three requirements that 'shall' be met to embrace the requirements of ISO 9001-2015
The statement includes a key word 'shall', meaning a command, an obligation required to be met.
With regards to creating or updating documented information the organisations 'shall' ensure appropriate;
•referencing convention is used so as to identify the documented information's date, title, author and / or reference number;
•format and media (the language, software version, graphics, which are held on paper, electronic);
Note: ISO 9000:2015 uses the word medium to define the technology used to hold information.
•review and approval for suitability and adequacy.
This clause is simply telling organisations they 'shall' ensure as far as documented information is concerned that it is appropriately identified, referenced, reviewed and approved for suitability and adequacy. This is nothing new and was previously covered by clauses 4.2.3 a to c for the documentation requirements of ISO 9001-2008.
